Downloading the Correct Claim Form 

There are generally 2 sets of Claim forms. One set is for Prudential Assurance Clients (aka Conventional) and Another set is for PruBSN Takaful Clients.

PruBSN Takaful Clients have their medical card start with the digit 9. If your card starts with 9 then click on the following links to download the appropriate forms

If you are at Non-Panel Hospital or need to Pay and Claim for Takaful you would need Dr to fill this up. Click here (note we don’t cover medical report fees so pls confirm with your Advisor if you need it)
